You can also make an in person appointment to apply for benefits.  If you choose not to apply online, at minimum, you should check out the disability checklist offered by the SSA on their website, and the disability report.  You will also want to print out the Authorization to Disclose Information form, to sign it, and bring it with you during your appointment.
